Clinical Case Management Specialist (Non-Medical Role)

In this role, you’ll be responsible for onboarding new clients, coordinating caregiver assignments, and maintaining strong relationships with clients, families, and our trusted community partners. This is a non-medical position, ideal for professionals with hands-on patient care experience (CNA, CCMA, MA, EMT, etc.) who want to step into a leadership and care coordination role.
🔍 Key Responsibilities:
Lead client onboarding and consult process
Act as a liaison between caregivers, clients, and community partners
Provide caregiver mentorship and back-up training support
Maintain up-to-date care plans and compliance documentation
Monitor care delivery via EHR dashboard & triage care concerns
✅ Requirements:
3+ years of caregiving or clinical experience (CNA, MA, EMT, LPN preferred)
Strong communication and tech skills (MS Office, CRM/EHR familiarity)
Bachelor’s in related field preferred but not required
Self-motivated, professional, and willing to travel up to 50%
💼 Compensation & Perks:
Hourly pay: $20–$27/hr DOE

Advanced Care Life Services (ACLS), Southern Oregon’s #1 in-home care agency, is growing its OSBN-approved education program and seeking an experienced Nursing Assistant Primary Instructor to lead with purpose and compassion.
This unique role blends clinical case management, RN-level assessments, and hands-on instruction in a classroom and lab setting. You’ll be shaping the next generation of CNAs while also supporting high-quality home care through care planning, delegation, and interdisciplinary coordination.
🔍 Key Responsibilities:
Educate and lead CNA students in OSBN-aligned curriculum and skills labs
Assess and develop care plans for senior clients in home care settings
Oversee caregiver delegation, compliance, and clinical documentation
Act as an RN liaison across internal teams, client families, and external providers
✅ Requirements:
Active RN License (Oregon or compact state)
3+ years clinical experience (home care or acute care preferred)

The Nurse / Clinical RN position
The Nurse / Clinical RN position will provide professional services and care to seniors through an interdisciplinary approach. Our independently Nurse owned business is looking to expand on our scope capabilities through consultation, assessment, planning, and evaluation by supporting the best possible outcomes for our seniors / clients.
This position will have knowledge on case management theory with a consultative approach for our clients/families. The ideal candidate will also have a passion for teaching and education. The Nurse / Clinical Educator position will be part-time in education and clinicals for our caregivers with an opportunity for additional hours with delegation, education, and senior support for our in – home agency. RN should be willing to work 5-9pm shifts and some weekends.
